Gateway Youth General Registration 2025/26

Welcome to Gateway Youth! We are glad you're here.

We require parents/guardians to fill out this general registration once a year to update contact information, consents, and our photo release.
It also explains our Code of Conduct.

Thank you for taking a few minutes to fill this out to stay connected, and so that we have the contact info we may need in the future.

Information received is confidential and is being gathered for the purposes of serving our youth while in the care of Gateway Church. The safety of your youth is our primary concern.
Precautions will be taken for their wellbeing and protection.

Gateway Church is collecting and retaining this personal information for the purpose of enrolling your youth in our programs, to assign the youth to the appropriate ministry, to develop and nurture ongoing relationships with you and your youth, and to inform you of program updates and upcoming opportunities at Gateway Church.
